CHARLESTON — State investigators reached no conclusions about what caused a fatal 2005 coal truck crash.
An Office of Miners’ Health, Safety and Training investigation report released Wednesday contains only facts about the accident, truck and 25-year-old driver Chad Cook, who died in the Nov. 8, 2005 wreck.
Cook’s death originally was attributed to a mining accident. The death was reclassified after government officials decided the wreck occurred on the property of a mine near the Grant-Tucker county line.
Federal inspectors concluded Cook lost control, probably from excessive speed.
State investigators also are scheduled to release a report on an independent contractor crushed to death by a metal plate that fell from a bulldozer last March.
